Identify the exact destination

Write the appointment building and entrance, not just the hospital name. Open the center's current directions or parking page. If instructions conflict, ask the center which one applies to the visit. Keep the relevant phone number or official contact route available for your trip.

Choose the first and last parts of the journey

A trip may include airport transport, hotel travel and movement between buildings. Check each part separately. Confirm whether a shuttle reaches your actual destination and whether it requires a reservation. Budget for a backup arrangement if the regular option is unavailable.

Investigate assistance early

Ask the center about transportation resources. ACS Road To Recovery is one possible starting point, with availability depending on local arrangements. The organization, not this website, determines whether it can help with a particular trip. [S20]

Ask about individual requirements

Tell the transport provider about access features you need and confirm what it can provide. Ask the care team about any travel instructions relevant to your care. Do not assume a volunteer ride, hotel shuttle or ride-hailing vehicle provides medical support.

Make the plan easy to hand over

Record pickup point, confirmed destination, provider contact and backup contact in the caregiver handoff sheet. Keep booking numbers and sensitive personal information out of public messages. Recheck the plan if the appointment location or expected timing changes.
