Start with the appointment confirmation

Write down the center name, complete appointment address and the date or arrival instructions supplied by the center. A familiar hospital name does not identify every building in its network. If multiple appointments are listed, ask whether they are at the same place and whether transport between them is needed. Keep the confirmation available offline.

Make a short list of questions

Ask the center what practical preparation it requires, how to contact the appropriate office if plans change, and where a caregiver should go. Follow the center's own clinical preparation instructions. This checklist does not tell you whether to fast, change medication or travel during treatment.

Plan lodging and transport together

Use the center's official travel page first. Ask about lodging assistance, hospital-arranged accommodation and patient rates. Once you know the likely destination, compare the room cost with parking, transportation, fees and cancellation terms. A lower nightly rate can still lead to a higher total trip cost.

Assign the practical tasks

Decide who confirms the appointment location, who handles reservations and who keeps the contact list. If someone is helping remotely, share only what they need to complete the task. Choose one person to maintain the latest version of the plan so conflicting reservations or old arrival details do not circulate.

Review before departure

Confirm the actual address, arrival method, lodging confirmation and any unresolved questions with the responsible organization. Download the appointment logistics planner and keep a copy with your travel documents. If a clinical or accessibility concern affects the plan, ask the care team or facility directly.
